The benefits of well designed Care Pathways / Patient Flow Plans have long been known.
However, when Focused On Health started to look at NHS care Pathways we found them to be very varied in quality, level of detail and level of obsolescence. They were often just long, flat, passive manual drawings. Since we needed to produce full, clear and concise definitions of how the various care functions worked, we had to produce our own tools to do this. The result was our PatientFlow Planning software.
As the following product overview explains, this is a system specifically tailored to NHS Needs giving all the tools needed to produce Care Pathway definitions using a desktop computer with only "drag and drop" skills needed.
Your PatientFlow Planning CD will include pre-built pathways for:
Pathways for the three broad categories of Emergency Admission, Post Elective Surgery Admission and Transfer In from another ICU. Plus there are supporting pathways for Transfer to HDU (or Ward), Transfer to another Hospital, Emergency Theatre and Emergency treatment in ICU, etc.
Separate pathways for Possible MI, Arrhythmia, LVF and Definite MI plus supporting pathways for insertion of Temporary and Permanent pacemakers, Discharge and Transfers.
Pathways for Diagnosis and Stabilisation, Bleeds & other GI Symtoms, Suspected and Diagnosed Cancers and Care of Elderly.
Pathways for Pre-Admission "Home-Making", Medical Examination, Planned and Emergency Surgical Admissions, Other Admissions, Fractures and non-invasive theatre, Discharges and Transfers etc.
Different pathways for Care in Ward (not needing Theatre), Emergency and Elective Admissions for Minor and Major Surgery (perhaps needing ICU) plus Discharge.
PatientFlow Plans are produced by every Client planning for a FlowModel collating all of the required data (patient arrivals, staff required, shift schedules, workload estimates etc). These FlowModels include: A&E + MAU, an Integrated Emergency Hub, 18-Week Orthopaedics, and Improving Access to Psychological Therapies (IAPT).
